What to Expect from Hospice
Sacred Mission uses a collaborative team approach to tailor a plan to meet the patient’s individual needs and wishes.
What is Hospice?
Hospice is a type of health care that focuses on comfort and management of a terminally ill patient’s symptoms and distress while attending and support their emotional, physical and spiritual needs during the final stages of life. Contrary to some misconceptions, hospice care prioritizes quality of life by reducing pain and managing other symptoms, allowing patients to enjoy time with their families in the comfort of their homes. This includes:
- 24/7 access to care
- Personalized care plan for patient’s diagnosis
- Medical care and evaluations by a physician and registered nurses
- Hospice aides to provide assistance with bathing and more
- Social workers to acts an advocated, advisor and guide for resources you may need outside of hospice care
- Spiritual care, counseling and support from a leader in your faith
- Grief and bereavement services
How Does it Work?
Hospice care provides a range of services, support and supplies to help patients during and through end-of-life care. This includes medication management for symptoms including pain, nausea and anxiety; equipment such as hospital beds, wheelchairs, oxygen or incontinence care; assistance navigating legal documents and medical bills; setting up wills and funeral arrangements; and offering grief and bereavement services for loved ones. Our team of physicians, nurses, social workers, aides and chaplains/spiritual leaders are available to collectively provide the emotional, physical and psychological support patients and their families need during this time.
