Our In-Home Hospice Care Service

Although hospice care is designed for end-of-life care, these services not only provide comfort for someone with a terminal diagnosis but offer support in the months prior to ensure patients can make the most of their time with their families and other loved ones in the comfort of their own home.

Our Services are Never Far Away

Entering hospice care for yourself or a loved one can be overwhelming. At Sacred Mission, we make this transition as easy as possible, working with you, your family and current physicians to ensure you receive the best care at every step.

Step 1

Once a referral has been made, our physician will come to your home to complete an evaluation to determine if the patient qualifies for hospice. If so, the physician will begin to build a plan of care around the diagnosis, answering any questions you and others may have.

Step 2

Your assigned RN will walk through the admission process with you and your family, explaining what medication and medical equipment will be ordered and brought into your home. We will help set up and organize your home to make care simple and accessible.

Step 3

Within five days of admission, a social worker and chaplain/spiritual leader will reach out to you and your family to start scheduled visits. These individuals can help with legal documents like wills, advance directives and other end-of-life decisions as well as provide spiritual and emotional support.

Step 4

Our team will work with you to schedule visits, starting with weekly nurse visits. The cadence of visits will depend on the physician’s orders and the progress of the patient over time, allowing you to enjoy time privately with your family and friends.
